NeuRoads NM utilizes structured, person-centered service models designed to promote functional rehabilitation and long-term independence. Each model reflects our commitment to individualized care across residential and community-based settings.

NeuRoads NM’s Service Model provides a structured continuum of care for individuals with acquired and traumatic brain injuries (ABI/TBI). Services are organized across residential and vocational pathways, allowing each participant to receive the level of support that aligns with their current needs and long-term goals.
Participants may enter the program at various levels of support. Residential options range from community-based rehabilitation homes to semi-independent and family-based living arrangements. As individuals gain skills and stability, supports may be adjusted to promote increased independence.
In addition to residential services, vocational supports such as job development and job coaching help participants build meaningful employment opportunities and strengthen community integration.
Our model is designed to be flexible, individualized, and progressive—supporting movement across levels of care as skills develop and goals evolve.

Change is often a constant for individuals living with a brain injury. Mood, social interactions, tolerance, environmental awareness, and relationship-building can all be affected.
NeuRoads NM provides structured strategies to strengthen psychosocial and functional skills, including social skills training, community integration, counseling referrals, cognitive behavioral training, and purposeful daily activities.
Our person-centered model places the individual at the center of care. Treatment plans and behavior strategies are developed collaboratively with the participant, family, support system, funder, and interdisciplinary team. Each activity is intentional and therapeutic, reinforcing positive behavioral growth and meaningful progress.
We are committed to supporting long-term reintegration and helping participants build a stable, independent future.

NeuRoads NM delivers rehabilitative services in the person’s own environment, making the process much more accessible and geared toward everyday life.
Through a multidisciplinary approach, therapeutic services, community integration, and vocational supports are coordinated to promote meaningful, measurable progress. Services may include physical, speech, and occupational therapies; nursing and behavioral supports; skills training; customized community supports; and employment services such as job development and job coaching.
Goals and objectives are designed as stepping stones toward increased independence and successful community reintegration. Each treatment plan is developed collaboratively with the participant, family, and interdisciplinary team to ensure care is individualized, purposeful, and aligned with long-term outcomes.
