The NeuRoads™ NM SMART Living Program empowers individuals to live safely, confidently, and independently at home and in the community. Our person-centered approach combines skill-building, guidance, and coordinated support to help participants reach their personal goals.
Certified Community Support Services (CCSS)
CCSS provides professional, community-based support to help individuals navigate daily life and access essential resources. Services include:
- Developing routines, organization, and time management
- Budgeting, shopping, and meal planning
- Emotional support and social connection
- Accessing housing, healthcare, education, and benefits
- Guidance with forms, appointments, and follow-ups
- Advocacy and collaboration with other professionals
- Goal setting and progress support
- Transition support for independence
Life Skills Coaching
Hands-on guidance for everyday independence:
- Communication, problem-solving, and decision-making
- Stress and time management
- Healthy habits and personal care
- Completing applications and paperwork
- Accompaniment to appointments and community activities
Homemaker Companion Services
Supportive, non-medical in-home assistance designed to promote independence, safety, and daily skill development:
- Home Management & Organization: Guidance with light housekeeping, tidying, and maintaining a comfortable living space.
- Nutrition & Meal Support: Assistance with meal planning, preparation, and grocery tasks, encouraging healthy eating habits.
- Routine & Schedule Assistance: Gentle reminders and structured support to help manage daily routines and appointments.
- Social Engagement & Companionship: Meaningful interaction to foster connection, learning, and mental well-being.
- Community & Outing Support: Accompaniment to appointments, social activities, or community events, promoting confidence and autonomy.
Housing Advocacy
Helping individuals secure and maintain safe, stable housing:
- Housing searches and lease guidance
- Landlord communication
- Facilitating application completion
Technology Mentoring
Teaching practical technology skills for daily life:
- Using phones, tablets, and computers
- Email setup, passwords, and apps
- Safe and responsible digital use
